Had Kevin Garnett got the max extension, he could've been making around $45Mil in a couple years. And if he got another max extension after that, he would be up over $50Mil. That would've been the best opportunity for a player to get paid that much, but since he took so much less it won't happen. And the only reason he could've got that much is because his previous contract was signed before a new CBA which limited those type of huge contracts.

He and Shaq were making more than the individual salary limit so they were exempt. Shaq was pulling $30 million a year with the Lakers for example.

Garnett's salary was one reason Marbury asked to be traded away. He would always be second best.

You can in theory make more than the cap, but you'd have to play for 20 years or so to make it happen. The salary cap has increased around 5% per annum while players are eligible for a 10% pay increase each year. Veteran players (7 years or more) are eligible for up to 30% of the cap plus 10% per year, so after 6 years they'd be making approximately 40% of the cap. Thats why I say that they lower the cap, allow one player to become a "franchise player" whose number doesn't count against the cap, and that's it.
